Your First Visit to the Office

Upon scheduling your appointment, we will send forms for you to fill out regarding your medical history. The information that you provide will enable us to better understand your symptoms and will make our appointment with you more effective.

You will also be sent a bladder diary that provides us with useful information regarding your drinking and urination habits, as well as any problem with leakage of urine that you might have. Please be sure to complete the new patient packet and bladder diary prior to the day of your appointment; the diary takes 24 hours to complete.
